The Health District’s Community Impact Team provides administrative and operational support to help coordinate this partnership.

Our commitment to health equity
Our plan places a strong emphasis on health equity because we believe everyone deserves care. To achieve this, the Alliance directs its energy toward four focus areas:
- Coordinating care so people experience smooth transitions between services.
- Improving data infrastructure to help us make informed decisions about community needs.
- Expanding clinical systems to increase access to medical treatment.
- Promoting behavioral health through nonclinical methods that reach community members where they live and work.
Your voice matters
Real change happens when community voices guide our work. Anyone in the community can help shape our priorities, identify gaps in care and contribute to meaningful solutions. We invite you to share your experiences so we can build a stronger support system for everyone.
