Making care accessible and affordable for our northern Larimer County neighbors—regardless of income, background or circumstances—is why we’re here.

As a taxpayer-funded health organization, our focus is simple: helping you get the care you need. Every decision we make starts and ends with what’s best for our community, including providing high-quality care, keeping services affordable and connecting you to resources that support your whole health.

Services and qualifications

Residents living within this geographic area may be eligible for Health District care. Our service area includes Fort Collins, Timnath, Laporte, Wellington, Bellvue, Livermore, Red Feather Lakes, Virginia Dale and far west neighborhoods of Windsor. The mountain communities of Stove Prairie, Poudre Park and Rustin all fall within the district.

Areas outside our boundaries include Loveland, Berthoud, Masonville, Drake, Glen Haven and Estes Park.

Our leadership and accountability

The community’s needs guide our Board of Directors, and our elected Board guides the Health District and a dedicated leadership team.

Our Board oversees our budget and policies. Our leadership team manages the daily programs that turn our mission into action.

We keep a public archive of meeting minutes and publish our schedule so you can stay informed.

To guide our work and plan for a healthier future, our Board develops a comprehensive strategic plan every three years. The current plan through 2027 focuses on four main priorities:
