We offer adult and children's dental services at 202 Bristlecone Drive. (next door to the Health District's
main office, 120 Bristlecone Drive, in north Fort Collins)
Who's eligible?
The Family Dental Clinic
serves residents of northern Larimer County who:

Meet financial
guidelines, and
-
Have no insurance or are
covered by Medicaid or CHP+ insurance.
If your income is low and
your assets are limited, you still may qualify for dental care on a
sliding-scale fee, even if you are not eligible for other programs such as
Medicaid or food stamps. Client assistance funds are very limited and we do
not have enough to help every one in need.
What services are available?
The Family Dental Clinic
provides dental care that meets the patient’s needs, including

Full dentures
-
Partial dentures
-
Root canals
-
Silver fillings
-
Tooth-colored fillings
-
We
do not provide orthodontic care
It’s important that
patients are happy with their mouth and smile. That means
relieving tooth
pain, repairing broken teeth and providing good preventive care. A dentist
determines the most appropriate care for each patient.
How do I apply ?
If you would like to see a dentist and are not an
established patient, please call the Eligibility Department at 407-7959 for
eligibility requirements. If you have a dental emergency, please call
contact the Family Dental Clinic 8 a.m.- 5 p.m.
Monday through Friday.
A limited number of emergency appointments are
available each day.
The clinic is very busy
and sees many patients each day. Patients are scheduled for appointments
based on the urgency of their need, as determined by the dentists. Patients are served as promptly as
possible. Please keep in mind that demand for services is high, which often
results in a longer waiting time. The Family Dental Clinic team works
closely with other dental professionals in the community to best meet the
needs of patients.

Become a Tooth Fairy for someone
in need
The
Tooth Fairy Fund for Grown-ups
is an effort to provide financial assistance to
adults who are unable to pay even the low sliding-scale fees
at the Health District’s Family Dental Clinic.A
local anonymous donor (our biggest Tooth Fairy so far) has
offered a $20,000 challenge grant for the
Tooth Fairy Fund
for Grown-ups.
If the Health District matches this donor’s $20,000 in 2009, we’ll
have $40,000 to help people in our community.
