CHP+ and Health First Colorado (Medicaid) Outreach Project
144 N. Mason Street, Unit 7
Fort Collins, CO 80524
The Health District helps families whose children are not covered with health insurance to obtain coverage through Child Health Plan Plus (CHP+) and Health First Colorado (Medicaid).
Child Health Plan Plus (CHP+) is a low-cost public health insurance for children ages 0-18 and pregnant women. A family of four can earn up to $63,960/year and still qualify. Enrollment is year-round.
Health First Colorado (Medicaid) is public health insurance for people who qualify. Individuals earning less than $16,040/year or a family of four earning less than $32,718/year may be eligible for Health First Colorado (Medicaid) coverage which now includes limited dental benefits for adults! Enrollment is year-round.
Health District staff are available to discuss public health insurance options with local families and assist them in enrolling, or re-enrolling, their eligible children in an appropriate program.
The service is designed to make it easier for eligible families in Larimer County to get health coverage and includes home visits, if needed, to help parents complete enrollment paperwork.
The CHP+/Health First Colorado (Medicaid) Outreach Project is funded by a grant from the Colorado Health Foundation.
Currently, enrollment services are provided by Larimer Health Connect, a new Health District program that also helps Larimer County residents choose and purchase private health insurance through the Connect for Health Colorado Marketplace. In-person assistance is available at five county locations. Se habla español.
