Feedback gathered during the discussions will be used for research purposes only as the Health District seeks to better understand the health, and health-care needs, of local residents.

The discussion groups are part of the Community Health Assessment, which the Health District has performed every three years since 1995. The assessment also includes a random survey of Larimer County households that is currently underway. Information gathered during the Community Health Assessment helps guide the planning and design of programs and services at the Health District. Results are also made available to other local organizations and agencies working on health-related issues.

People seeking more information about the Community Health Assessment can call the Health District at 970-224-5209 or visit www.healthdistrict.org/community-health-assessment. Those invited to take the health survey should visit www.larimercountysurvey.org.

The Health District is a public agency that provides residents of northern Larimer County with dental, mental health, preventive health and end-of-life planning services, in addition to connecting people to more affordable health insurance and prescription options.
