I would like to become a new adult patient of the Family Dental clinic and have the Family Dental Clinic be my “dental home” to take care of all my current and future dental needs, and I:

  • Am a resident of northern Larimer County

  • Believe I meet financial guidelines (income of 250% of Federal Poverty Level or less; and

  • Have Health First Colorado (Medicaid) or CHP+ or no commercial dental insurance.
