What is the registry called Colorado Immunization Information System?   The Colorado Immunization Information System (CIIS) is a computer system operated by the University of Colorado Health Sciences Center for the Colorado Department of Public Health and Environment under the Colorado Immunization Act. The registry helps doctors and nurses in Colorado keep track of the shots your child has received. A complete record of your child’s shots will help ensure that your child gets all recommended shots and does not get too many shots. 
How does the registry work?  

It keeps track of all the shots your child has received and who gave them.

It prints out a shot record for your child for school, daycare and camps.

 
How can the registry help you?   It keeps your child’s shot record in one place when your child goes to day care, school or camp.

It helps a doctor or nurse see what shots are needed to keep your child healthy.

It allows your doctor or nurse to remind you when it’s time for your child’s shot.

 

Why is this important?   The list of shots children should get to stay healthy and attend day care, schools and camps is getting more and more complicated. Keeping all the shots in one place helps your child to get one full set of recommended shots.

 

What are the Colorado school immunization requirements?   The State Board of Health decides which shots are required for school and day care. Shots currently required

A parent or guardian may exempt their child from any or all required shots based on personal or religious beliefs by signing an exemption form. A child may also be exempt from required shots for medical reasons.

 
What information about my child will be in the registry?   The shots your child has already received will be entered into the system. Shots your child receives in the future will also be entered. Other information will be included to be sure your child’s records are accurate and will not be confused with another child’s, such as: mother’s maiden name, child’s gender, and the office where each shot was given to your child. Your child’s address will be included to allow your doctor or nurse to send you a card to remind you when it’s time for your child to come in for a shot.
Who can see my child's shot information?   Information in the CIIS can be released only to:

The child’s parent

The child’s physician

A school or day care where the child is enrolled

A managed care organization or health insurer where the child is enrolled

Hospitals

Organizations that have an agreement with the State of Colorado under the Colorado Immunization Act

The Colorado Department of Health Care Policy and Financing for children enrolled in Medicaid.

 
Is the information confidential?   Under Colorado law, anyone who releases information in the CIIS to anyone who is not permitted to have the information commits a crime and can be punished. 
Can I see my child's shot information?   You can get a copy of your child’s record from your child’s doctor or nurse. If you need to make changes in your child’s record you can contact the CIIS at Colorado Immunization Information System, University of Colorado Health Sciences Center, P.O. Box 6508, F456, Aurora, CO 80045-0508
Can I decide that I don’t want to have my child’s shot information in the registry   You can choose at any time to have your child’s shots excluded from the CIIS. You need to complete a form that your doctor or nurse will give you.
